Jenn, an occupational therapist since 2012, possesses a wealth of experience in various settings including hospitals, home health services, and outpatient therapy clinics.
Her passion lies in assisting individuals with neurological conditions by enabling them to regain essential daily living skills and keeping her clients all living in their homes as they progress through life.
She is a Nationally Board-Certified Occupational Therapist. (NBCOT)
She is a Certified Aging In-Place Specialist and has completed over 1,050+ home evaluations aimed at reducing fall risk and supporting independent living at the client’s home. (CAPS)
She is a Certified Stroke Rehabilitation Specialist. (CSRS)
Former Certified Brain Injury Specialist. (CBIS)
She resides in Fort Wayne, Indiana with her partner and three dogs. She enjoys the lake, water skiing, sports, drawing/painting, concerts, spending time with friends/family, and being outdoors.
Home Evaluations
One passion area of mine is helping to create accessible home environments for people after injury, with disability, or to remain living at home while we age. I have been doing home evaluations for over 8 years and have completed over 1050 home assessments for adults of all ages.
I will start with a consultation phone call to discuss your needs followed by a home evaluation and follow up visits as needed.
Less than 1% of U.S. homes have the seven accessibility features to support aging in one’s home.
Accessible entrance.
Accessible and usable public and common areas.
Usable doorways (wide enough and handles).
An accessible route into and through the covered unit.
Light switches, electrical outlets, thermostats, and other environmental controls in accessible locations from a wheelchair.
Reinforced walls for grab bars.
Usable bathrooms and kitchens.

I focus on Universal Design and Visitability concepts.
Universal Design: A design technique that aims to create spaces that are accessible to people of all ages and abilities, regardless of their size, disability status, or other factors.
Visitability: Focus on new houses to have the following 3 features: One Zero-step entrance, doors with 32 inches of clear passage space, and at least one bathroom on the main floor that can be accessed with a wheelchair.

I offer comprehensive evaluations and tailored treatment plans aimed at addressing the specific areas necessary for you to engage in your desired sexual or intimate practices. Additionally, I provide consultation and counseling services to help you explore and problem-solve your unique situation, ensuring that you can navigate these important aspects of life with confidence and support.

Do you take my insurance?
Currently accepting Medicare and Traditional Indiana Medicaid!
I am also a cash pay practice for certain services. You will pay for the service prior to or at the time of the service. I will provide you with a superbill following the service that you may submit to your insurance company for reimbursement.
Do you have a building or office location?
There is some flexibility here. I am able to see clients in their homes within a specific radius from Fort Wayne, IN. I am also able to see clients within the community at various locations such as grocery stores, restaurants, certain pools etc… I do have access to a private office space. I also provide virtual therapy sessions and home evaluations.

Jennifer Ferguson has been our daughter Ashley’s occupational therapist for several years. Ashley was stricken by a rare Neuro-Autoimmune attack that had a profound impact on her motor skills. Jenn is continuously implementing new and innovative therapies to improve Ashley’s ability to become more independent. Ashley’s communication skills are also severely impacted. Jenn has the patience to ensure she is getting Ashley’s feedback on goals she is seeking to accomplish and addressing questions Ashley has during therapy.
